Reborn Form

Welcome to the St. Patrick Church Preparation Program for the Sacrament of Baptism.

This is a time of great joy for your family! We look forward to welcoming your child into the Catholic Church. Your preparation process will help you guide your child to a vibrant faith and life-long relationship with God.

 

Below you will find a step-by step list of what this journey will include. Parents will:

  1. Be registered members of St. Patrick Parish: to register, contact Amy in the parish office.

 

  1. Sign up for and attend “Reborn”: prior to the Sacrament of Baptism, parents will attend a three-evening course, Reborn. These classes are held for three consecutive weeks on Tuesday evenings. There are four sessions offered each year. Please return the attached registration form to Amy in the parish office.

 

  1. Select a Baptism date: On the final evening of Reborn, parents will complete the necessary paperwork. You may select a baptism date. Baptisms occur the first weekend of each month at the Saturday (4:30pm) and Sunday (7:30am, 9:30am and 11:20am) Masses.

 

  1. Return Godparent form: At Reborn, you will receive a form that your Godparents/Christian Witness will be required to sign. You are encouraged not to select Godparents until you have attended Reborn to make sure they meet the requirements for that role. All forms will need to be returned to the parish office no later than two weeks prior to the Baptism.
